- Original certificate of Four year grade average
- Original Birth certificate
- Employer approval
- Accredited copy of personal ID
- Master Certificate (for Doctorate registration)
- Military service certificate (for male)
- (6) personal photos
- Enrollment application with the name of Vice dean for graduate studies and research
